Here is the twenty-eighth round of modifications made to the JAC Safety Manual as per the JAC Safety Committee Meeting of 07-March-07.  Note: Modifications to the front and back covers will appear in the hardcopy versions only.

General Shop Policies

A.2.a.i

Move 1st sentence in parenthesis to ‘Supervisor’ heading:

  1. Supervisor (of a shop or any area where fixed or portable powered or un-powered machines and tools are located)

A.3.b

Remove  ,without the explicit authorization of the Facility Maintenance Manager,

  1. When machining operations are being undertaken which might pose a hazard to personnel passing through the workshops, signs will be posted on the doors to the workshops warning of hazardous work in progress.  Persons wishing to enter the shop area when these signs are posted, without the explicit authorization of the Facility Maintenance Manager, must stay within the yellow lined areas and wear safety glasses with side shields along with proper footwear as noted above.

A.3.i

Add to sentence: articles (e.g. lanyard and eyeglass retainers),

  1. Any employee using portable fixed tools (drill press, jig or band saw) must not wear loose articles (e.g. lanyard and eyeglass retainers), clothing or jewelry. Anyone with long hair must tie back the hair or wear acceptable hair protection while operating equipment.

  1. Air hoses must not be disconnected until they have been bled and pressure is securely turned off at its source and they have been bled.

Add to end of paragraph: Staff shall check the area to assure that all personnel have vacated.

  1. Staff working in the Mechanical Workshop or Helifier Bay shall vacate these areas when informed by the Administrative Department that the telephone system is being serviced or turned off. Staff shall check the area to assure that all personnel have vacated.

B.2.b.i

Change structure of sentence:

From: Inspects daily the powered industrial vehicle before it is used in accordance with section B5.

To: Inspects daily the powered industrial vehicle in accordance with section B5 before it is used.

6. OSHA 100 Hour Forklift Maintenance Inspection

The forklift 100 hour inspection is required and must be performed by a qualified mechanic in accordance with the instructions kept by the Facility Maintenance Manager.

C.4.a.ix

Remove from sentence: The operator shall

  1. The operator shall Observe whether there are any defects in the equipment during all operations, and stop and correct them as necessary.

C.4.b.iv

Remove last sentence (it is repeated in v.)

  1. The operator is responsible for operation of the crane or hoist. If required, an appointed person will direct the crane operation by approved hand signals. A stop signal shall be executed immediately no matter who gives it.

C.4.b.vi

Remove/add to sentence: cab operated/or hoist

  1. Before leaving a cab-operated crane or hoist unattended, the operator shall land any attached load, place controllers in the off position, and open the main line disconnect device of the specific crane.

Add new 2nd sentence:

  1. If power goes off during operation, the operator shall immediately place all controllers in the off position. As soon as practical, any suspended load shall be landed or secured. Prior to re-use of the crane, operating motions shall be checked for proper direction.

C.4.d.xi

Remove last sentence:

  1. Before personnel exit or enter a manbasket that is not landed, it shall be secured to the structure where the work is to be performed, unless securing it to the structure creates an unsafe situation. This procedure is an exception to the common working practices at JAC.

MK Safety Policies: JCMT and UKIRT

 

Note: the following addition was made following a special meeting held on 11-April-07

Add new paragraph f:
  1. If on arrival, and prior to communicating with the TSS/WL, the person encounters any hazardous situation, the person shall remove themselves to a safe location where he/she can contact the TSS/WL to receive instructions specific to that hazard.  If the person identifies a hazardous situation, at any time, that is not appropriately signed,they shall immediately inform the TSS/WL so that appropriate action can be taken.  Warning signs in use at JCMT and UKIRT are defined in Addendum 13 of the Safety Manual.

23. JCMT Observing Floor Drill Press The observing floor drill press is provided for use by staff who have limited machine tool clearance but need to be able to carry out small mechanical tasks. Authorised users are listed in Addendum 1. Note that, as with all machine tools, proper protective equipment such as eye protection shall be used

d.       Lockout/Tagout Interruption (Testing of Energized Equipment)

                                             i. In situations where the energy isolating device(s) is locked/tagged out and there is a need for testing or positioning of the equipment/process, the following sequence shall apply:

·     Clear equipment/process of tools and materials

·     Clear personnel

·     Clear the control of locks/tags  equipment of lockout/tagout devices according to established procedure

·     Proceed with test

·     De-energize all systems and re-lock/re-tag the controls lockout/tagout the equipment to continue the work

Appendix 3 B 1. & 2.

Replace: ‘identify locked out system’ with locked out system identity

 

Note that the lockout systems use "Slave" keys and "Master" keys. To use them, remove the master key from the lower switch and insert it into the master/slave unit above. Then one or more of the slave keys can be taken out. The master key cannot be returned to the master switch until all slave keys are returned. Put the master castell key into the multiple lockout device and take one of the slave keys to the control room, hang the key on the lockout board and write your name, date, and identify locked out system on the lockout board locked out system identity.

4.       If, on the other hand, your work is such that ANY movement of the antenna could cause a hazard (e.g., inspecting the rollers), then it is a wise precaution  necessary to lock the antenna to the carousel with a suitably placed clamp. Consider also applying the carousel brakes in case the wind should blow the building around.
